The safety and welfare of our Scouts campers is our #1 priority.
Camp Kingsley has a medical officer on-site during camp weeks. We also only employ trained and certified lifeguards and CPR-trained staff. You can rest assured that your child is in a safe environment while having the time of their lives.
medical forms required for camp
In order for youth, parents and leaders to attend overnight camp, BSA requires an up-to-date medical form to be submitted. Physical exams and forms cannot be more than one year old. Click the button below to download the most current BSA medical form and instructions. Once completed (including all parts, A,B, and C) please submit the medical forms to your unit leader.
PARENTS PLEASE PAY SPECIAL ATTENTION:
Anyone that does not have a COMPLETED HEALTH FORM, which would include the Dr.’s signature on part C form, will NOT be allowed into camp. There are NO exceptions to this policy and it is STRICTLY enforced.

Dietary Needs Form (if special meals are needed)
Authorization for Camper Release (if parent or guardian is not picking Scout up)
Early Release Form (if Scout is leaving camp early)
